Getting Started
This application will take about sixty (60) minutes to complete.
Basic Requirements:
  • Vehicle Identification Number (VIN)
  • your base’s TLC license number
  • an address that is not a P.O. Box
  • a valid email address
  • a debit card, credit card or checking account
  • a social security number or employer ID number
Application Fees:
  • $75 DMV Inspection fee for vehicles with 500 or more miles
  • $800 CMVT tax for vehicles registered outside of New York City
  • $550 for a two year license
    (TLC is waiving the $550 license fee for wheelchair accessible For-Hire-Vehicles.)
Restrictions:
  • Vehicle cannot be changed until the vehicle you applied with passes the inspection
  • If you must replace your vehicle after you receive your TLC decals,you must replace the vehicle with the same vehicle type you appiled with (electric to electric, wheelchair to wheelchair)
  • Battery electric vehicle licenses for individual drivers must be registered under driver's personal name and social security number
  • Vehicle licenses issued for use with SHL permit must stay affiliated to SHL permit
Required Documents:
After completing this online form, and submitting payment, you will need to upload the following documents using nyc.gov/tlcup. You will be sent additional information about this when you have successfully submitted this application. You may not submit documents in person at the TLC
Individuals, Corporations and Partnerships:
  • Government-issued photo ID of applicant, all corporate principals, shareholders and/or partners
  • For-Hire Insurance Certificate (FH-1)
  • Certificate of Liability – except for Paratransit
  • Excess Insurance Policy – if applicable
  • One of the following in applicant’s name:
    • DMV registration (must submit for vehicles registered outside of NY)
    • Vehicle Certificate of title or
    • Vehicle Certificate of origin and
    • Bill of Sale or
    • Vehicle Leasing Agreement
Corporation applicants ONLY:
  • Certificate of incorporation
  • Corporate minutes listing the names and shares of officers or members
  • an IRS issued CP-575 Notice or a 147-C letter
Partnership applicants ONLY:
  • Certificate of partnership
LLC applicants ONLY:
  • Articles of Organization
  • Operation Agreement
  • List of Members

Note: Election of Officers forms are not acceptable

Applicant's name must be identical on all documents.
The address on each document you provide must match one of the addresses you list on the application.
*Wheelchair Accessible Vehicles - TLC may require documentation from the up-fitter or modifier at any stage of the licensure process.
